Community Habilitation

Our Community Habilitation program provides support services within an individual’s private home or living arrangement or in the community.

Our primary goal is to improve a person’s ability to live independently and fully utilize community resources. For example

  • We encourage individuals to strive to live independently in the community with minimal staff support.

  • With increased independence as the goal, we work with individuals to develop life skills that include shopping, cooking, doing laundry, maintaining personal hygiene, tidying and cleaning the home environment, and taking mass transit.

  • Once programming plans are developed with individual and family input, trained staff assists the person in attaining their desired outcomes.

  • This service offered by IRI is intended to supplement the care being given by family members and, where applicable, coordinate this service with other existing services.

  • Individuals typically receive several hours of this service each week.
